    Issue 1 Proposed Tax Levy (Additional) -- City of Norwood (Majority Approval Required)
    An additional tax for the benefit of the City of Norwood, County of Hamilton, Ohio, for the purpose of CONSTRUCTION, RECONSTRUCTION, RESURFACING, AND REPAIR OF STREETS, ROADS, AND BRIDGES at a rate not exceeding two (2) mills for each one dollar of valuation, which amounts to twenty cents ($0.20) for each one hundred dollars of valuation, for five (5) years, commencing in 2013, first due in calendar year 2014.

    Issue 2 Proposed Tax Levy (Renewal) -- Village of Silverton (Majority Approval Required)
    A renewal of a tax for the benefit of the Village of Silverton, County of Hamilton, Ohio, for the purpose of CURRENT EXPENSES at a rate not exceeding five (5) mills for each one dollar of valuation, which amounts to fifty cents ($0.50) for each one hundred dollars of valuation, for four (4) years, commencing in 2013, first due in calendar year 2014.

    Issue 3 Proposed Tax Levy (Additional) -- Delhi Township (Majority Approval Required)
    An additional tax for the benefit of Delhi Township, County of Hamilton, Ohio, for the purpose of PARKS AND RECREATIONAL PURPOSES at a rate not exceeding seventy-five hundredths (0.75) mill for each one dollar of valuation, which amounts to seven and five-tenths cents ($0.075) for each one hundred dollars of valuation, for a continuing period of time, commencing in 2013, first due in calendar year 2014.

    Issue 4 Proposed Tax Levy (Additional) -- Delhi Township (Majority Approval Required)
    An additional tax for the benefit of Delhi Township, County of Hamilton, Ohio, for the purpose of PROVIDING AND MAINTAINING FIRE APPARATUS, APPLIANCES, BUILDINGS, OR SITES THEREFOR, OR SOURCES OF WATER SUPPLY AND MATERIALS THEREFOR, OR THE ESTABLISHMENT AND MAINTENANCE OF LINES OF FIRE ALARM TELEGRAPH, OR THE PAYMENT OF FIREFIGHTING COMPANIES OR PERMANENT, PART-TIME, OR VOLUNTEER FIREFIGHTING, EMERGENCY MEDICAL SERVICE, ADMINISTRATIVE, OR COMMUNICATIONS PERSONNEL TO OPERATE THE SAME, INCLUDING THE PAYMENT OF ANY EMPLOYER CONTRIBUTIONS REQUIRED FOR SUCH PERSONNEL UNDER SECTION 145.48 OR 742.34 OF THE REVISED CODE, OR THE PURCHASE OF AMBULANCE EQUIPMENT, OR THE PROVISION OF AMBULANCE, PARAMEDIC, OR OTHER EMERGENCY MEDICAL SERVICES OPERATED BY A FIRE DEPARTMENT OR FIREFIGHTING COMPANY at a rate not exceeding one and seventy-five hundredths (1.75) mills for each one dollar of valuation, which amounts to seventeen and five-tenths cents ($0.175) for each one hundred dollars of valuation, for a continuing period of time, commencing in 2013, first due in calendar year 2014.

    Issue 5 Proposed Resolution 2013-10 and Contract -- Sycamore Township (Majority Approval Required)
    Shall the resolution of the Board of Township Trustees of the Township of Sycamore approving the contract with the City of Madeira, Ohio, for the designation of the Sycamore Township JEDZ Kenwood-East Joint Economic Development Zone be approved?

    Issue 6 Proposed Resolution 2013-11 and Contract -- Sycamore Township (Majority Approval Required)
    Shall the resolution of the Board of Township Trustees of the Township of Sycamore approving the contract with the City of Madeira, Ohio, for the designation of the Sycamore Township JEDZ Kenwood-Central Joint Economic Development Zone be approved?

    Issue 7 Proposed Resolution 2013-12 and Contract -- Sycamore Township (Majority Approval Required)
    Shall the resolution of the Board of Township Trustees of the Township of Sycamore approving the contract with the Village of Amberley, Ohio, for the designation of the Sycamore Township JEDZ Kenwood-Southwest Joint Economic Development Zone be approved?

    Issue 8 Proposed Tax Levy (Additional) Current Operating Expenses -- Milford Exempted Village School District (Majority Approval Required)
    An additional tax for the benefit of Milford Exempted Village School District, Counties of Clermont and Hamilton, Ohio, for the purpose of CURRENT OPERATING EXPENSES at a rate not exceeding four and five-tenths (4.5) mills for each one dollar of valuation, which amounts to forty-five cents ($0.45) for each one hundred dollars of valuation, for a continuing period of time, commencing in 2013, first due in calendar year 2014.

    Issue 9 Proposed Bond Issue -- Forest Hills Local School District (Majority Approval Required)
    Shall bonds be issued by the Forest Hills Local School District, Counties of Hamilton and Clermont, Ohio, for the purpose of NEW CONSTRUCTION, IMPROVEMENTS, RENOVATIONS AND ADDITIONS TO SCHOOL FACILITIES AND PROVIDING EQUIPMENT, FURNISHINGS AND SITE IMPROVEMENTS THEREFOR, INCLUDING SAFETY AND SECURITY IMPROVEMENTS, BUILDING ACCESSIBILITY IMPROVEMENTS, ENHANCEMENTS TO THE LEARNING ENVIRONMENT AND CAPITALIZED INTEREST in the principal amount of $47,000,000 to be repaid annually over a maximum period of thirty-five (35) years, and an annual levy of property taxes be made outside the ten-mill limitation, estimated by the county auditor to average over the repayment period of the bond issue one and eighty-six hundredths (1.86) mills for each one dollar of tax valuation, which amounts to eighteen and six-tenths cents ($0.186) for each one hundred dollars of tax valuation, commencing in 2013, first due in calendar year 2014, to pay the annual debt charges on the bonds, and to pay debt charges on any notes issued in anticipation of those bonds?

    Issue 10 Proposed Tax Levy (Additional) -- Oak Hills Local School District (Majority Approval Required)
    Shall a levy be imposed by the Oak Hills Local School District, County of Hamilton, Ohio, for the purpose of AVOIDING AN OPERATING DEFICIT in the sum of $5,275,000 per year, and a levy of taxes to be made outside of the ten-mill limitation estimated by the county auditor to average four and eighty-two hundredths (4.82) mills for each one dollar of valuation, which amounts to forty-eight and two-tenths cents ($0.482) for each one hundred dollars of valuation, for a period of five (5) years, commencing in 2013, first due in calendar year 2014?

    Issue 11 Proposed Tax Levy (Additional) -- Deer Park Silverton Joint Fire District (Majority Approval Required)

    Issue 12 Proposed Tax Levy (Renewal and Increase) -- Miami Township Waste District (Majority Approval Required)
    A renewal of 1.25 mills and an increase of 1 mill to constitute a tax for the benefit of Miami Township, County of Hamilton, Ohio, for the purpose of WASTE COLLECTION AND DISPOSAL at a rate not exceeding two and twenty-five hundredths (2.25) mills for each one dollar of valuation, which amounts to twenty-two and five-tenths cents ($0.225) for each one hundred dollars of valuation, for five (5) years, commencing in 2013, first due in calendar year 2014.
